Xanthelasma Removal

Xanthelasma palpebrarum is a common skin condition characterised by yellowish, soft, flat plaques that appear on or around the upper and lower eyelids.

These patches are made up of cholesterol deposits and can range from small, subtle areas to larger, more noticeable discolouration or thickening.

Although xanthelasma is not harmful or painful, it may:

  • Affect cosmetic appearance and self-confidence
  • Slowly increase in size over time
  • Indicate an underlying lipid imbalance in some patients (though not always)

Xanthelasma are yellowish cholesterol-rich deposits that form within the skin. Although they are most typically found around the inner corners of the eyelids, they can be widespread.

At Dr. Jenny Clinic, we offer surgical excision of xanthelasma under local anaesthetic.

This approach offers the best chance of complete removal at the time of surgery, especially with deeper plaques. We do always have to warn however, that due to the nature of xanthelasma, even after successful removal your body can create new deposits over time.

We take extra care to preserve eyelid contour and minimise the visibility of your scarring by placing the stitches along the natural skin folds where possible.

If you have multiple lesions affecting multiple lids we will evaluate how many areas can safely be treated in one sitting. It may be safest to stage the excision, not just to optimise your recovery but also to minimise scarring.

Because in some individuals, xanthelasma can be associated with abnormal cholesterol levels in the blood, we always advise that it is important undergo some blood testing with your GP prior to embarking on treatments. This not only helps you to proactively screen for any underlying metabolic or cardiovascular risk factors but it also helps us determine whether you are at higher risk of recurrence.

Following your consultation, you will be consented for your procedure.

The lesion(s) to be removed is photographed for your medical records.

The area is then numbed using a numbing injection.

The operative area is then cleansed with an antiseptic solution and the surrounding area is covered with a sterile sheet or drape to ensure that infection risk is minimised.

The lesion is then surgically removed and the treatment area is stitched closed. We use removable sutures which we will remove in the week after surgery.

The wound will be dressed with a thick layer of antibacterial eye ointment.

Most procedures take between 30-60 minutes and you can return to normal activities shortly afterwards, however we do advise that you take it very easy in the days post treatment to minimise swelling.

It is adviseable that someone accompanies you back home after the procedure in case the post-operative swelling or wound oitment impair your visibility.

Your aftercare advice is an important step to ensuring optimal healing.

  • Keep the area dry for 48 hours.
  • You will experience swelling, tightness and bruising in the area which may take 5-7 days to fully resolve.
  • Gentle cold compresses may be used to alleviate swelling.
  • Vaseline can be used as a wound “dressing” until the sutures are removed.
  • We will be bringing you back to the clinic for removal of your sutures within 5-7 days from your procedure.
  • Avoid strenuous activity, swimming, or saunas for 2 weeks.
  • Apply high-factor SPF to the healing area to minimise pigmentation and support scar healing.

We understand that our patients can be very busy with their daily lives and therefore it is ideal for many to be able to combine their consultation and surgery during the same visit.

Therefore, although we do offer the option to proceed with surgery on the same day (known as a ‘See and Treat’ appointment), we will only do so if you are certain you would like to proceed and if it is safe to proceed on the same day.

If you are interested in same day treatment please choose a treatment appointment or a “consultation and treatment” appointment when booking so that adequate time can be allocated.

If you would prefer to think through all the treatment options first and make provisions for potential downtime and restrictions then it is best for us to carry out your consultation first, then plan a separate visit for your treatment.

As an ethical medical clinic, we never want you to feel pressured to undergo treatment on the same day, therefore rest assured that your consultation fee is fully redeemable against your treatment, even if you decide to delay your treatment by several weeks or months.

As we always advise patients in the clinic, never feel rushed to commit to an elective, non-emergency procedure and take all the time you need to make that decision.
